Transition: Student Role

Transition is a process of preparing students with disabilities for a successful adulthood by identifying opportunities, teaching skills for independence, and connecting to adult community agencies.

After High School

  • What kind of job do I want?
  • Where can I go for job training?
  • How do I find a Job?
  • Where will I live?
  • How can I go to college?
  • How will I get around in the community?
  • What community agencies can help me?

Graduation will be here before you know it! There are many decisions, choices, and opportunities in your future that await you. Sooner or later you will be thinking about all the questions above. Don’t you think it would be smart to look at opportunities and start to make plans now, instead of waiting for graduation day? Don’t just sit and wait for something to happen! YOU can make it happen! That’s Transition Planning!
